Here you can give your power of attorney to another person in connection with your case in case of certain critical illnesses and/or payment by reduced earning capacity and exemption from payment. You must have the proxy holder's name, CPR- or CVR number, address and, if applicable, phone number and email address ready when you fill in the power of attorney.
